For all horses, to be completed by the applicant

Justification of Value:
For all horses when the amount of insurance is higher than the horse’s original purchase price and for horses which were Home Bred; to be completed by the applicant

Veterinary Certificate:
For all horses valued over $50,000 and/or horses that have been treated for an accident, illness or disease in the last twelve months. Also for horses under 12 months and over 15 years of age. To be completed by a licensed veterinarian.
